Artykuł ma na celu opis specyfiki zaburzeń językowych i poznawczych oraz zmian w zachowaniu i osobowości w otępieniu semantycznym (SD). SD, znane również jako wariant semantyczny afazji pierwotnej postępującej (svPPA), jest postępującym zaburzeniem neurodegeneracyjnym, charakteryzującym się utratą pamięci semantycznej. Pacjenci z SD mają zazwyczaj trudności z aktualizacją wcześniej znanych wyrazów oraz rozpoznawaniem znajomych obiektów i twarzy. Objawy kliniczne obejmują anomię, wielomodalne zaburzenia rozumienia znaczeń wyrazów, afazję z zachowaną płynnością mowy i wzrokową agnozję asocjacyjną. Mowa pacjentów z SD charakteryzuje się pauzami niezbędnymi do znalezienia brakujących wyrazów, zmniejszoną częstotliwością występowania wyrazów samoznaczących, obecnością parafazji semantycznych, zwiększonym stosunkiem czasowników do rzeczowników oraz wielością powtórzeń. Wraz z postępem choroby zmiany zachowania i osobowości są często postrzegane jako podobne do tych obserwowanych w otępieniu czołowo-skroniowym.

The aim of the article is to describe the specificity of language, cognitive, behavioral and personality changes in semantic dementia (SD). SD, also known as semantic variant primary progressive aphasia (svPPA), is a progressive neurodegenerative disorder characterized by loss of semantic memory. SD patients generally have difficulty in generating familiar words and in recognizing familiar objects and faces. Clinical signs include anomia, impaired comprehension of word meaning, fluent aphasia, and associative visual agnosia. Speech of SD patients is marked by wordfinding pauses, reduced frequency of content words, semantic paraphasias, increased ratios of verbs to nouns, and multiple repeats. As the disease progresses, behavioral and personality changes are often seen similar to those seen in frontotemporal dementia.
