The MHD-DCP is a conceptual model that enables one to identify and explains the causes and consequences of disease, trauma, and disruptions to the development of a person or a population. It demonstrates that social participation is not only the result of our identity, our choices, impairments to our organs, and our abilities and inabilities, but also of characteristics of our living environment. For example, an individual’s preparing meals or going to regular school may be influenced by the reinforcement of that person’s capabilities and alteration of his or her functional limitations through rehabilitation, as well as through the reduction of obstacles caused by prejudices, lack of assistance or resources, and absence of accessibility within the home, school or urban space.

MHD-DCP jest modelem pojęciowym, który pozwala zidentyfikować i wyjaśnić przyczyny oraz skutki choroby, urazu i zakłóceń w rozwoju jednostki lub populacji. Model ten pokazuje, że uczestniczenie społeczne nie jest tylko wynikiem naszej tożsamości, naszych wyborów, upośledzenia naszych narządów oraz naszych zdolności lub ich braku, lecz również właściwości naszego środowiska życiowego. Na przykład, na przygotowywanie posiłków lub uczęszczanie do szkoły przez określoną jednostkę można wpłynąć, wzmacniając zdolności tej jednostki oraz modyfikując jego lub jej ograniczenia funkcjonalne za pomocą rehabilitacji, jak również poprzez redukcję przeszkód będących konsekwencją przesądów, braku wsparcia lub zasobów oraz nieistnienia środków dostępu w domu, w szkole, a także w przestrzeni miejskiej.
