Equality in the access to health service is one of major human rights, and providing it is the basic aim of every health care system. Changes which are taking place in Poland - migration processes, the aging society with simultaneous strong demographic differentiation in regional cross-section, restructuring of medical facilities etc., cause changes in accessibility to medical facilities. For the last twenty years the average life span of the Lodz voivodeship inhabitants has been on the lowest level in Poland. The research objective of this paper is to find an answer to the question whether the relatively short life span of Lodz voivodeship inhabitants is influenced by limited access to medical infrastructure. The article presents the essence of the problem of accessibility to health protection and indicates the measures of its assessment: subjective - the assessment of accessibility carried out by patients based on the survey (based on social diagnosis) and objective – including the number of hospitals and hospital beds per 10 000 inhabitants, the number of primary health care clinics and dental clinics per 10 000 inhabitants, advice provided per capita and the number of people per public pharmacy (based on Central Statistical Office data and Health Care Clinics registers). The accessibility to medical infrastructure analysis in the Lodz voivodeship has been presented against the background of Poland. The carried out analysis does not enable a positive verification of the thesis that the relatively short life span of Lodz voivodeship inhabitants is af- fected by limited access to health care infrastructure.

Monitoring of the setting changes in environment, to adapt them to one's activities, is the basic premise of functioning of present economic subjects. A wider glance at the environment where the enterprise operates is assured - so-called the analysis of teams - proposed by M. E. Porter. Clusters - are groups of mutually related and interested in a definite field of enterprises (producing final articles or providing service), specialized tradesmen, points of service, financial institution, firms acting in related sectors and connected competitive institutions. Recognition of component part of the team makes it possible for the enterprise to make best decisions, aiming at creating and improving competitive position on the market. This article tests the identification of components of a team in theoretical and empirical (approach example of health service centre) as well as the influence of analysis of the clusters onto building a competitive position of the subject.

Restructuring problem of health care establishment - in sense of liquidation of ineffectiveness establishments or cost-generating without any adequate results - is discussed for more than 15 years, but the pressure of health circles on maintenance status quo (first of all because of protection of employees) was stronger than economic account. But changes - in spite of resistance of the circle - in the staff, material resources and their allocation are happen and it is worth to evaluate their level (for examples for purpose directions of further changes in this area). The aim of this article is to define: - changes in medical care's organization (with taking into account ownership structure of health care establishment), - differentiation of employment in medical care in Poland in voivodship structure.
