Współcześnie znaczące, a w praktyce często bezsprzecznie stosowane cztery pryncypia etyki medycznej (autonomia, nieszkodzenie, opieka, sprawiedliwość) w dyskursie etycznym ciągle na nowo napotykają na krytyczne głosy, w których dąży się do powrotu do jednolitego pryncypium etycznego. Autor niniejszego artykułu podejmuje wnikliwą próbę uczynienia z pryncypium proporcjonalności, które zawsze stanowiło podstawę praktyki medycznej i pielęgniarskiej i wynika z zasady niewyrządzania szkody, pryncypium podstawowego. Kryterium proporcjonalności jest przy tym ujęciem, które w sposób systematycznyrekonstruuje cztery pryncypia w ich współzależności, a w przypadku ich zastosowania w praktyce podsuwa rozwiązania dla trudności z tym związanych.

The four principles of medical ethics (autonomy, non-maleficence, beneficence, justice), which are influential today and often received unquestioningly in practice, have repeatedly encountered critical inquiries in the ethical discourse, in which the return to a uniform basic ethical principle is demanded. In this article, an attempt is made to make the principle of proportionality, which has always been the basis of medical and nursing practice and which also results from the principle of non-maleficence itself, comprehensible as such a basic principle. The proportionality criterion proves to be an approach to systematically reconstruct the four principles in their context and to develop solutions for many of the difficultiesassociated with their application in practice.

Die heute einflussreichen, in der Praxis oft unhinterfragt rezipierten vier medizinethischen Prinzipien (Autonomie, Nicht-schaden, Fürsorge, Gerechtigkeit) sind im ethischen Diskurs immer wieder auch auf kritische Anfragen gestoßen, in denen doch wieder die Rückführung auf ein einheitliches ethisches Grundprinzip gefordert wird. Im vorliegenden Beitrag wird versucht, das Prinzip der Verhältnismäßigkeit, das ärztlicher und pflegerischer Praxis immer schon zu Grunde liegt und das sich auch aus dem Nichtschadens-Prinzip selbst ergibt, als ein solches Grundprinzip einsichtig zu machen. Das Verhältnismäßigkeitskriterium erweist sich dabei als Ansatz, die vier Prinzipien systematisch in ihrem Zusammenhang zu rekonstruieren und für viele mit ihrer Anwendung in der Praxis verbundene Schwierigkeiten Lösungsansätze zu entwickeln.
