Scientific, objective approach to consciousness has allowed to obtain some experimental data concerning brain activity, ignoring, however, the longstanding philosophical tradition. Spectacular development of neuroscience which has been observed recently made this dissonance particularly noticeable. The paper addresses the main problems of discrepancy between neurobiological research and philosophical perspective. Current opinions concerning neural correlates and models of consciousness are discussed, as well as the problems of working memory, attention, self, and disorders of consciousness. A new neurobiological approach to describe brain function in terms of brain connectivity (so-called connectome) is also presented. Finally, the need to introduce at least some aspects of philosophical approach directly into neurobiological research of consciousness is postulated.

Neurological early and long-term rehabilitation plays a crucial role in the therapy of patients with disorders of consciousness (DOC) such as unresponsive wakefulness syndrome or minimally conscious state. Neuroscience tries to explain the effect of music therapy on all levels of the nervous system = activity in patients with DOC, but full understanding is still incomplete. This paper attempts to answer how current clinical outcomes may reflect the influence of various factors including music's capacity. Based on their interdisciplinary perspective and previous experiences, the authors try to investigate the extent to which current occupations have been explored. The authors analyzed the literature data concerning the results of the studies published until the first half of 2016, to sum up the current state of research. Research in the main databases: PubMed, PEDro, Health Source: Nursing/Academic Edition was made using specified keywords and inclusion and exclusion criteria. Next, the authors sorted them all out into a coherent view of the current state. Music listening may constitute a part of an enriched environment setting. However, due to weak evidence, the therapeutic value of music-based interventions in patients with DOC is uneven or limited. The role of music therapy is thus complementary. Standardized clinical settings, protocols, and behavioral measures should be developed to increase its clinical validity, reliability, sensitivity, and objectivity. There is a reasonable hypothesis that music may produce a high level of diagnostic and therapeutic outcomes as stimuli usually reflecting strong personal meaning in patients with DOC.

The aim of this paper is to describe problems related to diagnosis of disorders of consciousness. That is being done with emphasize on its complexity by which diffi- culty for physicians and neurologists in monitoring subject’s health rises. Valuation of persons in coma, vegetative state – unresponsive wakefulness syndrome – and minimally conscious state turns out to be related with presence of executive and cognitive functions, such as attention, memory, integration of sensory information, command-following and language. Alongside difficulty in diagnosis, a new perspective emerges for how to think about consciousness. Just like we look at how patients behave in the clinical context we can look for situations from everyday life when we notice that someone is conscious.

Celem artykułu jest dyskusja wybranych problemów związanych z diagno- styką globalnych zaburzeń świadomości. Pokazuje się, że jednym z zasadniczych źródeł trudności diagnostycznych, z którymi mierzą się lekarze monitorujący stan pacjentów jest złożoność takich globalnych zaburzeń. Ocena tych stanów u osób w śpiączce, stanie wegetatywnym czy minimalnych stanach świadomości ma również charakter globalny i składają się na nią charakterystyki kompetencji wykonawczych i poznawczych, takich jak uwaga, pamięć, integracja multisensoryczna, wypełnianie poleceń, czy język. Oprócz problemów narzędzi diagnostycznych, pojawia się nowa perspektywa myślenia o świa- domości. Nie w kategoriach rozważań pojęciowych oderwanych od realnych przypadków, w tym od zaburzeń świadomości, a w postaci usytuowania rozważań nad świadomością w intersubiektywnej wiedzy o jej przejawach i zaburzeniach.
