The paper presents a description of speech disorders, in particular those concerning phoneme realization. The author provides a review of different methods in describing speech defects with regard to phoneme realization and she points out to pros and cons of these methods. She also formulates her own proposal for the description of speech impediments taking into account all phonetic categories.

Mutism in Autism. A Case Study

An increase in recent years in the number of children diagnosed with mutism, i.e. silent period/dumbness, as well as an increase in the number of children with autism spectrum developmental disorders, may be related to civilizational transformations that determine the biological and social conditions for the child’s development. Autism as a neurodevelopmental disorder is differentiated from mutism – a symptom of social phobia. Diagnosing of the two disorders is the domain of psychiatrists and psychologists; however, the treatment of children with autism and mutism is increasingly often provided by logopedists (speech therapists). The goal of the article is to present the pathomechanisms and symptoms of mutism in the context of autistic disorders and the procedure for logopedic management in the case of a 9-year-old autistic child with developed language skills, in whom mutism occurred as a result of having experienced a trauma. Establishing of the criteria for the differential diagnosis of autism and mutism and developing a strategy for the treatment of the child with combined communication skills disorders requires taking into account the neurobiological pathomechanisms underlying these symptoms.

Zwiększanie się w ostatnich latach liczby dzieci, u których rozpoznawany jest mutyzm, czyli zamilknięcie, a także wzrost liczby dzieci z całościowymi zaburzeniami rozwojowymi ze spektrum autyzmu mogą mieć związek z przemianami cywilizacyjnymi, determinującymi biologiczne i społeczne warunki rozwoju dziecka. Autyzm, jako zaburzenie neurorozwojowe, jest różnicowany z mutyzmem – przejawem fobii społecznej. Diagnozowanie obydwu tych zaburzeń jest domeną psychiatrów i psychologów, jednak terapią dzieci z autyzmem i mutyzmem coraz częściej zajmują się logopedzi. Artykuł ma na celu zaprezentowanie patomechanizmów i symptomów mutyzmu w kontekście zaburzeń autystycznych oraz procedury postępowania logopedycznego w przypadku 9-letniego dziecka autystycznego z rozwiniętymi sprawnościami językowymi, u którego w wyniku przeżytej traumy wystąpił mutyzm. Ustalenie kryteriów diagnozy różnicowej autyzmu i mutyzmu oraz wypracowanie strategii terapii dziecka ze sprzężonymi zaburzeniami sprawności komunikacyjnych wymaga uwzględnienia neurobiologicznych patomechanizmów, leżących u podłoża tych objawów.
