Objectives To study any possible correlation between arsenic toxicity and the development of oral carcinoma in West Bengal population. Material and Methods Ethical clearance for this study was obtained from the Vivekananda Institute of Medical Sciences. Out of 30 785 patients attending our hospital from November 2012 to July 2015, 107 cases and 50 control individuals were selected. The hair and buccal smear samples were obtained upon their consent for the purpose of the analysis of arsenic count and cytogenetic damage, respectively. Results Ninety-six percent of cases came from the highly arsenic affected districts and 81.3% showed their arsenic count above the safe limit (0.8 μg/g) whereas 96% of the controls’ arsenic count was within the safe limit. The study showed a significant difference of the micronuclei and apoptosis frequency between the cases and controls. Conclusions The difference of micronuclei and apoptosis frequency between cases and controls was significant. The maximum number of cases came from highly arsenic affected areas and a higher percentage of cases showed elevated arsenic count, as compared to controls, which may indicate a possible link between arsenic toxicity and this disease. Rozważano konsekwencje przecięcia mięśnia okrężnego ust u 286 chorych leczonych operacyjnie z powodu raka jamy ustnej. Izolowane przerwanie ciągłości tkanek otoczenia szpary ust, bez utraty ich podparcia kostnego, powodowało przejściowe zaburzenia funkcji pokarmowych, artykulacyjnych i estetycznych. Wycięcie wraz z guzem nowotworowym kości wchodzących w skład szkieletu części twarzowej czaszki trwale zmieniało uwarunkowania przestrzenne szpary ust i ograniczało możliwości kompensacyjnego tworzenia mowy. Zaburzenia funkcji wargi po wycięciu guza nowotworowego w niewielkim stopniu wpływały na artykulację. Wycięcie całej wargi i odtworzenie jej złożonym płatem tkankowym przemieszczonym z odległego miejsca ograniczało możliwości tworzenia fonemów wargowych. Udział tkanek otoczenia szpary ust w kompensacji funkcji jamy ustnej, upośledzonych po wycięciu nowotworu, podkreśla znaczenie pooperacyjnej sprawności tego narządu.

The article considers the consequences of the section of the orbicular muscle of the mouth in 286 patients treated surgically for oral cancer. Isolated disruption of tissues surrounding the mouth slit, without the loss of their bone support, resulted in a temporary dysfunction of the alimentary, articulatory and aesthetic functions. The excision of the malignant tumor, along with the bones constituting the skeleton of the facial part of the skull, permanently altered the spatial conditions of the mouth slit and limited the possibilities of the compensatory production of speech. Malfunctioning of the lip after the neoplastic tumor excision only slightly affected the articulation. Excision of the entire lip and its reconstruction from a compound tissue flap displaced from a remote site limited the ability to produce labial phonemes. The participation of the tissues surrounding the slit in the compensation of the oral cavity function, impaired after the excision of the tumor, stresses the importance of the postoperative performance of this organ.
