Artykuł jest próbą przedstawienia wstępnych wyników testowania modelu wsparcia dzieci i rodzin w ramach projektu Deinstytucjonalizacja szansą na dobrą zmianę – projekt zintegrowany. Dzielnica Bielany m.st. Warszawy, który jest realizowany w ramach Projektu Operacyjnego Wiedza–Edukacja–Rozwój, Działanie 4.1. Innowacje społeczne. W projekcie Uniwersytet Kardynała Stefana Wyszyńskiego w Warszawie powołał podmiot leczniczy EZRA UKSW, który testuje rozwiązania związane z koordynacją i współpracą instytucjonalną dotyczącą leczenia zaburzeń psychicznych dzieci i młodzieży. Innowacją wdrożoną w prezentowanym podejściu do organizacji opieki nad niepełnoletnim i jego rodziną jest systemowa praca z siecią oparcia społecznego oraz konsekwentne szukanie zasobów we wspieraniu procesów zdrowienia.

The article is an attempt to present the preliminary results of testing the model of support for children and families in the framework of the project ‘Deinstitutionalisation as a chance for good change – integrated project. Bielany District of the Capital City of Warsaw’, which is part of the ‘Operational Project Knowledge – Education – Development, Measure 4. 1. Social innovation’. In the project, Cardinal Stefan Wyszyński University in Warsaw established a therapeutic entity EZRA UKSW, which tests solutions related to coordination and institutional cooperation in the treatment of mental disorders of children and young people. The innovation implemented in the presented approach to the organization of care for a minor and his family is the systematic work with the social support network and consistent search for resources in supporting the processes of health.
