W artykule jest przedstawiona opieka społeczna dla osób starszych zorganizowana na podstawie zintegrowanego modelu działań opiekuńczych różnych podmiotów świadczących usługi na rzecz seniora. Interdyscyplinarna współpraca jest prezentowana na przykładzie dwóch działów środowiskowej opieki społecznej w Anglii: Reablement Team oraz EMI – Staying at Home Scheme. Omawiane działy w swoim funkcjonowaniu kierują się podejściem skoncentrowanym na osobie oraz dążeniem do maksymalizowania niezależności i samodzielności osoby starszej.

The paper describes social care services for the elderly based on integrated model of activities undertaken by various professionals involved in care provision of an elderly person. Reablement Team and EMI – Staying at Home Scheme are two examples of Home Care services involving interdisciplinary co-operation in social care and health provision. The core value of these two care services are Person Centred Approach and maximising independency of an elderly person.
