Introduction. Human T-cell lymphotropic virus types 1 and 2 (HTLV-1 & 2) are frequent co-pathogens among immunosuppressed individuals, particularly HIV/AIDS infected persons. Dual infected persons usually present with false normal or high CD4+ T cells count as a result of the ability of HTLV to induce clonal proliferation of CD4+ T lymphocytes. There is paucity of information on this clinical entity in Nigeria. Aim. This study aimed to determine the seroprevalence of HTLV-1/2 and associated cellular immune response among antiretroviral naïve and experienced HIV infected persons at Bauchi State, Nigeria. Material and methods. One hundred and eighty two (182) HIV seropositive patients’ blood samples were analyzed for anti HTLV-1/2 IgM and IgG antibodies using ELISA while CD4+ T cells were counted using Flow cytometry technique. Socio-demographic data of the subjects and clinical history were obtained via questionnaire and medical records, respectively. Results. The seroprevalence of anti-HTLV-1/2 was 14%. This comprised 76 (41.8%) males and 106 (58.2%) females. Six (3%) were seropositive for both ant-HTLV -1&2 IgM and IgG. Of the total positive for anti-HTLV-1/2, 20 (25%) ART-naïve and 6(5.9%) ART-experience subjects. Whole blood CD4+ T cell count was significantly high in HTLV-1/-2 IgG/IgM seropositive subjects compared to their HTLV-1/-2 negative counterpart. Conclusion. All subjects (100%) who were HTLV-1/-2/HIV co-infected had normal to higher CD4+ T cell counts. It is suggested to be very careful in using only CD4+ counts to monitor HIV progression or as indicators for ART.

Kvalita života HIV pozitivních osob

The study aims to investigate factors that infuence the quality of life of HIV infected people. Based on findigns from a number of researches high level of CD4+ lymphocytes count, low viral load, employment and social support have been found to have a positive effect on the quality of life of HIV infected individuals, while depression, low CD4+ lymphocytes count, high level of viraemia, and comorbidities (such as pneumocystis pneumonia, hepatitis C, cytomegalovirus infection) have a negative effect on the quality of life. The role of gender in the quality of life of HIV infected people is as yet not clear. Some researches document lower quality of life in HIV positive women compared to HIV positive men, while others don’t show any difference. Antiretroviral therapy has both positive and negative effect on quality of life. There is an indication that co-infection with hepatitis G virus may positively influence health related aspects of the quality of life of HIV infected people. In a further research, the authors will examine the effects of HGV infection on general quality of life in HIV positive individuals. As yet very little research has been done in this area. It is hoped that by discovering the range of factors that may influence the quality of life of HIV infected people, their needs will be better understood, which may in turn increase their adherence to antiretroviral therapy and lower their tendency towards risky behavior.

Studie si klade za cíl prozkoumat faktory, které ovlivňují kvalitu ţivota HIV infikovaných osob. Na základě výsledků mnoha výzkumů byl zjištěn pozitivní efekt na kvalitu ţivota HIV infikovaných osob u těchto faktorů: vysoká hladina CD4+ lymfocytů, nízká virová náloţ, zaměstnanost a sociální opora. Zatímco deprese, nízký počet CD4+ lymfocytů, vysoká virémie a souběţná onemocnění (jako např. pneumocystová pneumonie, hepatitida C a cytomegalovirová infekce) mají negativní efekt na kvalitu ţivota. Úloha genderu v oblasti kvality ţivota HIV infikovaných osob není zatím zřejmá. Některé výzkumy dokládají niţší kvalitu ţivota HIV pozitivních ţen v porovnání s HIV pozitivními muţi, zatímco jiné neprokazují ţádné rozdíly. Antiretrovirová terapie má jak pozitivní, tak negativní efekt na kvalitu ţivota. Objevují se zjištění, ţe souběţná infekce virem HGV můţe mít pozitivní vliv na se zdravím související kvalitu ţivota HIV infikovaných osob. V dalším výzkumu budou autoři zkoumat efekt infekce virem HGV na celkovou kvalitu ţivota HIV pozitivních osob. Zatím v této oblasti bylo podniknuto málo výzkumů. Objevení faktorů, které ovlivňují kvalitu ţivota HIV infikovaných osob, můţe zvýšit porozumění jejich potřebám, coţ můţe zvýšit jejich adherenci k antiretrovirové terapii a sníţit tendence k rizikovému chování.

The main goal of the article is to analyze the problems and needs of individuals and families living with HIV/AIDS, and to identify characteristics of effective social and psychological support for representatives of the target group. Basic methods are: analysis of research and practice working with PLHIV, theoretical data generalization, synthesis features to determine social and psychological support for individuals and families living with HIV/AIDS in Ukraine. In the article we reitemized the basic aspects that determine the vulnerability of individuals and families (acceptance of the diagnosis and adherence to treatment, stigma and discrimination, self-stigma, drug addiction, domestic violence and other difficult life circumstances); were singled out typical problems and difficulties of HIV-infected people, their basic needs. The main objective of social and psychological support for individuals and families affected by HIV/AIDS was defined as improving their quality of life, protection of rights and interests, and combating further spread of HIV. Basic measures, the implementation of which has proved their effectiveness, were detailed: counseling individuals and families living with HIV/AIDS on a wide range of health; assistance in making status and/or disclosure of a child or family status; home patronage, and if it’s necessary – medical support; protection of rights and representation; organization of self-help groups and so on. Some characteristics of social and psychological support for individuals and families living with HIV/AIDS were determined. Materials of the article can be useful for scientists and practitioners who work in the provision of PLHIV. Based on the analysis of the theory and practice of socio-psychological support for individuals and families living with HIV/AIDS, we can conclude that the activities are aimed both at expanding the resources of the client and his family, and in the whole community, where PLHIV lives, and are focused on overcoming obstacles on the formation of adherence to treatment, overcoming self-stigma, improving the quality of life with HIV infection; effective support which is possible only through an integrated approach to problem solving and meeting the client’s needs and involves social workers, psychologists, doctors, lawyers – that is implemented in multidisciplinary team of specialists; to achieve high efficiency of such work is to use the fullest possible range of measures of social and psychological support of PLHIV and to consider the features of implementation of such activities.
