The Role of Endoscopy in the Assessment of the Vocal Tract and Proper Speech Acquisition in Children with Cleft Palate

Speech reception is a perceptual phenomenon. Hence in the assessment of cleft palate speech its auditory evaluation is a basic golden standard with respect to identification of speech disorder and defining the degree of its intensity. Yet auditory evaluation does not provide information about the cause of this disorder. And here, with a good knowledge of the cleft palate issues and perfect skills in using the endoscopic technique, nasofiberoscopy, i.e. endoscopy using flexible optics, is irreplaceable. It enables direct visualization of the velopharyngeal sphincter at rest and during speech, allowing the simultaneous assessment of its structure and function. It also allows simultaneous endoscopic evaluation of other elements of the vocal tract that exert a direct effect on speech resonance. Thus it has fundamental significance for the choice of the treatment procedures and for the evaluation of the successive stages of treatment and rehabilitation.

Rozszczep podśluzówkowy podniebienia jest specyficzną postacią wady rozszczepowej podniebienia, stwarzającą znaczne trudności diagnostyczne i lecznicze. Może on mieć formę jawną, manifestującą się charakterystyczną triadą objawów, oraz ukrytą, która jest szczególnie trudna do zdiagnozowania. Wiele z tych rozszczepów przebiega bezobjawowo. Często ujawniają się one u dzieci po usunięciu trzeciego migdałka w postaci niespodziewanie pojawiającego się po zabiegu nosowania otwartego o bardzo różnym nasileniu. W pracy przedstawione są podstawowe cechy różnicujące tego typu rozszczepy i przydatność diagnostyki endoskopowej dla postawienia prawidłowego rozpoznania. Dyskutowane są również wskazania do operacji oraz uwarunkowania konieczności wykonania operacji naprawczej podniebienia przed ewentualnym usprawnianiem logopedycznym.

The submucous cleft palate is a specific form of the cleft palate defect, which is highly difficult to diagnose and treat. It may have an overt form, manifesting itself in a characteristic triad of symptoms, and an occult one, which is especially difficult to diagnose. Many of these palatal clefts produce no symptoms. They often become manifested in children after adenoidectomy in the form of unexpected hypernasality of varying intensity after surgery. The study presents basic distinctive features of this type of cleft palate and usefulness of endoscopic diagnostics for making a correct diagnosis. It also discusses indications for surgery and determinants of the need to perform a palate repair before possible logopedic rehabilitation.

Odbiór mowy jest zjawiskiem percepcyjnym. Stąd także w ocenie mowy „rozszczepowej” podstawą i niezastąpionym złotym standardem jest jej ocena odsłuchowa. Pozwala ona najlepiej na zidentyfikowanie zaburzenia mowy i określenie stopnia jego nasilenia. Ocena odsłuchowa nie dostarcza jednak informacji o przyczynie tego zaburzenia. I tu, przy dobrej znajomości problematyki rozszczepowej i opanowanej do perfekcji umiejętności posługiwania się techniką endoskopową nazofiberoskopia, czyli endoskopia przy użyciu optyki giętkiej, jest niezastąpiona. Umożliwia ona bowiem bezpośrednią wizualizację zwieracza podniebienno-gardłowego w spoczynku i w mowie, pozwalając na jednoczesną ocenę jego struktury i funkcji. Pozwala także na jednoczesną endoskopową ocenę innych elementów traktu wokalnego, mających bezpośredni wpływ na rezonans mowy. Ma więc zasadnicze znaczenie dla wyboru postępowania leczniczego i oceny jego wyników na poszczególnych etapach leczenia i usprawniania.
