Autorki przedstawiają pewną transformację logopedycznego myślenia na temat ankyloglosji – sposobu badania wędzidełka języka i postępowania w przypadku jego skrócenia, a także różne opinie na temat ankyloglosji, jakie prezentują logopedzi w Polsce. W opracowaniu zaprezentowana została także propozycja typologii skróconych wędzidełek języka, będąca uzupełnieniem testu B. Ostapiuk. Autorki wskazują także problemy dotyczące ankyloglosji, które – według nich – powinny zostać rozwiązane.

The authors present some transformation of logopeadic thinking about ankyloglossia – concerning the methods of lingual frenulum examination, treatment in case of its shortening as well as different opinions expressed by speech therapists in Poland. A suggestion of a new typology of abnormal lingual frenulums, which is complementary for Ostapiuk’s test, has been presented. The authors also point further problems with ankyloglossia phenomenon, which according to them should be resolved.

This article discusses issues related to ankyloglossia. The author points to a different approach between different scientific disciplines concerning shortened frenulum. When examining the various classifications of the ankyloglossia, the author pays special attention to the symptoms of the disturbances in the function of the oral cavity and the coexisting articulating abnormalities.

W artykule omówiono zagadnienia dotyczące ankyloglosji. Autorka wskazuje na odmienne podejście różnych dyscyplin naukowych do tematu skróconego wędzidełka języka. Analizując rozmaite klasyfikacje ankyloglosji, zwraca szczególną uwagę na związane z ankyloglosją zaburzenia funkcji kompleksu ustno-twarzowego oraz współwystępujące nieprawidłowości artykulacyjne.

W artykule przedstawiam głoskę jako trójwymiarową kompozycję narządów tworzących obwodowy aparat wymowy (w kierunku poziomym do przodu – do tyłu w odniesieniu do równoległych płaszczyzn pionowych czołowych w kolejnych odcinkach kanału głosowego, w kierunku pionowym w górę – w dół w odniesieniu do równoległych płaszczyzn poziomych, w kierunku poziomym w prawo – w lewo w odniesieniu do płaszczyzny pionowej sagitalnej). Omawiam przede wszystkim (ilustrowane fotografiami z wideofonicznych nagrań) nietypowe artykulacyjne pozycje i ruchy warg, języka, żuchwy w odniesieniu do płaszczyzny pionowej sagitalnej. Artykulacyjną asymetrię języka wiążę bezpośrednio z ograniczeniem ruchomości języka w ankyloglosji. Asymetrię warg (żuchwy) wiążę z kompleksem labiowelarnym i interpretuję jako pośredni skutek ankyloglosji, powstający w wyniku kompensacji niwelującej ograniczenia ruchomości języka w tworzeniu głosek. Kwestionuję strategię wspomagania języka wargami w logopedycznej terapii u osób z ankyloglosją.

In this article I am presenting the sound as a three-dimensional composition of organs forming the peripheral pronunciation apparatus (horizontally, to the front-to the back with reference to parallel frontal vertical planes in subsequent segments of voice channel, vertically up-down with reference to parallel horizontal planes, horizontally, to the rightto the left with reference to the hori zontal, sagittal plane). First of all , I am discussing the untypical articulatory positions and movements of the lips, tongue and mandible (illustrated by photographs from videophone recordings) with reference to the vertical sagittal plane. I am relating the articulatory asymmetry of the tongue directly to the limited mobility of the tongue in ankyloglossia. I am relating asymmetry of the lips (mandible) to the labiovelar complex and interpreting as an indirect effect of ankyloglossia, caused by compensation that levels the restricted tongue mobility in making sounds. I am challenging the strategy of supporting the tongue by lips in speech therapy of persons with ankyloglossia.

Celem pracy jest opis postępowania diagnostycznego oraz sprzężonego postępowania terapeutycznego będącego wynikiem połączenia emisji głosu, fizjoterapii oraz terapii logopedycznej w przypadku czynnościowej dysfonii hiperfunkcjonalnej. W pracy przedstawiono rolę fizjoterapeuty i logopedy w interdyscyplinarnej terapii zaburzeń głosu. Opisano wizualne i palpacyjne techniki diagnostyczne i terapeutyczne, które mogą być stosowane przez terapeutę głosu. Opisano zasady i elementy emisji głosu, które powinny być stosowane podczas terapii czynnościowej dysfonii hiperfunkcjonalnej.

The aim of the work is a description of diagnostig management and coupled theraphy which is a result of conecting voice emission, physiotheraphy and speech theraphy in case of hyperfunctional dysphonia. The paper presents a role of physiotherapist and speech therapist in interdisciplinary theraphy of voice disorders. There is described visual and palpatory diagnostic and terapeutic techniques which can be used by voice therapist. There is described rules and elements of voice emision which should be used during therapy of hyperfunctional dysphonia.

In the paper, I discuss the short frenulum of the tongue as a multidisciplinary research problem and changes in the approach to the treatment of ankyloglossia in practice. I refer to selected diagnostic and therapeutic issues related to the consequences of ankyloglossia (also in terms of prevention), especially those that are the subject of inter- and intradisciplinary disputes between specialists. Attention is focused on the presentation of the differences in the approach to assessing the quality of pronunciation in relation to ankyloglossic dysfunction of the tongue, seeing them as the main source of misunderstandings between specialists in the assessment of indications for the surgical treatment of ankyloglossia (frenotomy, frenectomy of the tongue).

W opracowaniu omawiam krótkie wędzidełko języka jako wielodyscyplinarny problem badawczy oraz zmiany w podejściu do leczenia ankyloglosji w praktyce. Odnoszę się do wybranych zagadnień diagnostyczno-terapeutycznych związanych z następstwami ankyloglosji (także w aspekcie profilaktyki), zwłaszcza tymi, które stanowią przedmiot między- i wewnątrzdyscyplinarnych sporów między specjalistami. Uwagę skupiam na przedstawieniu różnic, które dotyczą podejścia do oceny jakości wymowy w odniesieniu do ankyloglosyjnej dysfunkcji języka, w nich upatrując główne źródło nieporozumień między specjalistami w ocenie wskazań do chirurgicznego leczenia ankyloglosji (frenotomia, frenectomia języka).
