Nasal polyps are soft, pink or pearl white outgrowths usually connected with nasal mucosa by flaccid crus. A histological image of nasal polyps indicates significant differences in particular cases. Because of the variety of nasal polyps, the choice of proper laryngological treatment is very difficult. Difficulties in treatment are due to the lack of consensus among physicians regarding the etiopathogenesis of nasal polyps. Probably etiopathogenesis of nasal polyps is not homogeneous and polyps’ formation is influenced by many coexisting mechanisms.

Purpose: Certain risk factors of urolithiasis may be diagnosed in 75% of children with stones. They include genetic, anatomic, metabolic and nutrition causes. The diagnostics' evaluation should determine and rule out anatomic factors and establish metabolic disorders. The treatment is based on an increased fluid intake, and alterations in the nutrition pattern. The aim was to analyze the clinical course of urolithiasis in children below 3, and detect the possible factors affecting the success of therapy. Materials and methods: Between 2009 and 2013, we retrospectively investigated the cases of 68 children (34 boys and 34 girls) under 3 years who were treated in the clinic. Urolithiasis was diagnosed at the mean age of 10 months; the average follow-up lasted 26 months until reaching 3 years. The retrospective analysis was based on medical documentation and direct contact with parents. Results: Among the risk factors, the most common was hyperciuria. The treatment of children was based on an increased daily fluid intake and dietary recommendations. Among the specific treatments, the most common were citrates (51%). In the majority of cases, the therapy lasted over a year after the clinical absence of stones. This therapy was successful in 62% of children. The factors contributing to the success of the treatment were: a negative family history of urolithiasis, unilateral stones, normal urinalysis, decrease of the Ca/Cr index, and compliance of dietary and medical recommendations. Conclusions: The treatment is based on the elimination of risk factors, and alterations in the dietary pattern. High percentage of parents did not respect the medical recommendations.

Introduction. Laryngeal squamous cell carcinoma (LSCC) is a common type of head and neck malignancy. Because of unsatisfactory results of therapy, development of new strategies for LSCC treatment is needed. It is believed that heat shock protein 70 (HSP70) is involved in pathogenesis of LSCC. Thus, targeting HSP70 seems to be promising strategy for laryngeal cancer treatment. Aim. The aim of the study was to assess the HSP70 concentration in laryngeal squamous cell carcinoma specimens and its correlation with tumor volume and TNM staging. Material and methods. An ELISA method and a Bradford protein assay were used to evaluate the HSP70 concentration in peripheral blood cells, tumor tissue and lymph nodes from the patients suffering from LSCC. Results. We demonstrated that the HSP70 concentration is significantly different between examined compartments. The highest level was observed in peripheral blood, while the lowest was in the lymph nodes. The HSP70 expression was correlated to tumor volume. Conclusion. Our results showed varied expression of HSP70 in tissue from patients with LSCC, but there was no association between HSP70 concentration and TNM staging. Currently, application of HSP70 inhibition as a LSCC treatment could be rather associated with systemic blocking of this molecule than target inhibition in tumor tissue. However, further analysis on a larger group of patients is needed.

This article is focused on eating disorders (ED) as a research topic in Slovak and Czech scientific publications. The aim of this study was to thematically analyze the development of theory and research in relation to treatment research field of ED for the past 30 years in the two above mentioned countries and identify the current research trends and their applications. 58 articles were analyzed out of 8 psychology journals and 3 yearbooks published between 1993 and 2014. The main research method was the thematic analysis. The themes and discourses were divided into two main areas – the changing discourse on eating disorders and applications in the areas of different treatment and prevention interventions. Both main theme were further divided in subtopics – theories of eating disorders (definition of eating disorders, comorbidity, explanation of occurrence and theoretical context), application (different forms of intervention, focuses and promising opportunities). According to researched articles ED are nowadays seen more as a spectrum of symptoms than a two different faces of the illness (originally anorexia and bulimia was recognized as two main forms). Considering treatment possibilities, in majority CBT was used, however this approach went through different modifications (CAT – cognitive-analytical therapy, DBT- dialectical behavioral therapy), as well as it is used in combination with other methods, e.g. medical treatment. Complex treatment programs specialized on eating disorders are being introduced and different types of treatment facilities are described. Family therapy and its variant – multiple family therapy seem to be the latest trend in Czech republic, but also articles about motivational interview and externalization technique appeared. The important change in area of treatment and prevention appeared with internet and online communication. While advancement in communication technologies brings new possibilities for early detection, prevention and treatment of ED, it also brings the problem of pro-anorectic websites and the problem of lay support through forums and blogs, which while intended to help sometimes brings unproven information and implicitly support pro-disorder eating behavior and attitudes, the trend commented in newer analyzed publications. Analysis of published texts shows that the development of discourse on etiology, classification treatment is in accordance with international trends, there are some specifics mainly in area of treatment programs.

Introduction. Head and neck haemangiomas occur quite rarely. These are benign lesions, often involving the pharynx and larynx. They almost always pose a diagnostic and therapeutic problem. Case description. This paper describes the case of an asymptomatic internal carotid artery aneurysm manifested by a throat tumour. A 78-year old woman with sudden deafness and vertigo was additionally diagnosed with a throat tumour. After radiological diagnosis of a haemangioma, the patient refused surgery. This article presents diagnostic methods and various types of treatments for head and neck haemangiomas. Attention was paid to the possibility of serious complications when deciding on surgical intervention of haemangiomas without a confirmed diagnosis. Conclusion. Suspicion of head and neck haemangiomas necessitates using all available diagnostic methods. The treatment plan should take into account the pace of progression, the patient’s age and co-morbid conditions. .

Influenza is an acute infectious disease of the airways that occurs in every age group. The course of the disease may have a different severity and clinical picture, from asymptomatic to severe with symptoms of respiratory failure. The possibilities of pandemic infections and serious complications throughout the course of influenza distinguish it from other viral respiratory diseases, prompting us to treat it in a special way. Diagnosis of the disease is usually clinical in nature, and additional studies may be helpful in clinically relevant cases. The drug recommended for treatment and chemoprophylaxis is oseltamivir. Clinical benefits are greatest if treatment starts as early as possible, within 48 hours of the first symptoms of influenza. Clinical studies indicate that early treatment with oseltamivir may shorten the duration of the disease and reduce the risk of its complications. Annual influenza vaccination is the most effective method of preventing this disease, and the only contraindication to vaccination is the occurrence of an anaphylactic or other severe allergic reaction after a previous vaccination

Diet is a major environmental factor which maintains metabolic balance and body homeostasis. The aim of the study was to present the latest reports on the biological activity of the food compounds in a critical review. It has been shown that bioactive substances supplied with food e.g. polyphenols, flavonoids or phenolic acids exhibit immunomodulatory and anti-inflammatory fun-ctions. What is more, they favourably downregulate major oncogenes and enhance suppressors’ expression. Studies show that these substances can be an important component of cancer prevention and treatment. However, there are micronutrients like iron or copper which elevated levels in cancer cells occurs and should be considered as a therapeutic targets.

The problem of search of an authenticity of the cities looks very actual and draws attention of numerous researchers. In the article the analogy between the human person and the city in this problem is used. For the person, as well as for the city, awareness of own identity is necessary to resist depressive tendencies. It is shown that symptoms of depression of the person are similar to the ones for depression of the cities. The identity of the city necessary for opposition of a depression is offered to be looked for in two planes – historical and geographical (to be looked for in historical and geographical plane). The historical plane is made by local myths and legends of the city and its inhabitants. The geographical plane of identity is concentrated on features of relief, climate, flora and fauna of that region in which the city is located.

Background:Musician's dystonia is a task-specific movement disorder which manifests itself as a loss of voluntary motor control in extensively trained movements. In many cases, the disorder terminates the careers of affected musicians. Approximately 1% of all professional musicians are affected.Etiology and Pathophysiology: The pathophysiology of the disorder is still unclear. Findings include (a) reduced inhibition in different levels of the central nervous system, (b) maladaptive plasticity, e.g. in the somatosensory cortex and in the basal ganglia, and (c) alterations in sensorimotor processing. Epidemiological data demonstrated a higher risk for those musicians who play instruments requiring maximal fine-motor skills. For instruments where workload differs across hands, focal dystonia appears more often in the more intensely used hand. In psychological studies, musicians with dystonia had more perfectionist tendencies than healthy musicians. These findings strengthen the assumption that behavioral factors may be involved in the etiology of musician's dystonia. Hereditary factors may play a greater role than previously assumed. Pre-liminary findings suggest a genetic contribution to focal task-specific dystonia with phenotypic variations including musician's dystonia.Treatment:Treatment options for musician's dystonia include pharmacological interventions such as administration of Trihexyphenidyl or Botulinum Toxin-A as well as retraining programs and ergonomic changes in the instrument. A long-term follow-up study was performed in 144 patients with musician's dystonia. The out-come was revealed on average 8.4 years after onset of symptoms. Outcome was assessed by patients' subjective rating of cumulative treatment response and response to individual therapies. Seventy-seven patients (54%) reported an alleviation of symptoms: 33% of the patients with Trihexyphenidyl, 49% with Botulinum Tox-in, 50% with pedagogical retraining, 56% with unmonitored technical exercises, and 63% with ergonomic changes. In embouchure dystonia, only 15% of patients reported improvement. The results demonstrate that the situation of musicians with focal hand dystonia may be significantly improved. Positive results after retraining and un-monitored technical exercises underline the benefit of an active involvement of patients in the treatment process. Only exceptionally, however, can musicians with focal dystonia return to normal motor control using the currently available therapies.

The main points are related to the cultural-anthropological (Michel Foucault) and theological contextualization of diseases (Jean-Claude Larchet) and their treatment in the Middle Ages. Based on the South Slavonic hagiographic literature, the terms physician and healer are defined and specified. The study focuses on the mental disease (insanity), which according to the methodology of Larchet is three types: somatic nature of madness, the madness of demonic origin, and madness of spiritual origin. Also partly concerns the problem of God fools’ insanity.

Monitorowanie skuteczności leczenia przeciwwirusowego oceną wiremii HBV DNA metodą PCR , ilościowego oznaczania HBsAg, HBeAg i kinetyki spadku tych stężeń umożliwia modyfikację leczenia w trakcie jego trwania. Dalej istnieje potrzeba selekcjonowania nowych czynników pozwalających przewidywać skuteczność prowadzonego leczenia przeciwwirusowego u zakażonych HBV w celu ograniczenia ryzyka rozwoju odległych powikłań, tj. rozwoju marskości wątroby ze wszystkimi następstwami, oraz raka wątrobowokomórkowego

A large number of people infected with hepatitis B virus in Poland and the world as well as limited availability and efficiency of causal therapy necessitate further research on predictors of response to the therapy. It has to provide choosing the best medication and capability to modify the therapy. The paper discusses predictors of responses to the HBV therapy. Factors associated with response to PEG-IFN therapy are: low HBV DNA level, HBeAg presence, high Al at activity and active inflammation in histopathological examination of the liver biopsy. Modification of the therapy is possible by monitoring its effectiveness by using the assessment of HBV DNA levels with PCR method, HBsAg quantification and kinetics of the decrease in their concentrations. There is still a need to discover new factors that will let predict the efficiency of the antiviral therapy in HBV positive patients to decrease the risk of developing the long-term complications such as liver cirrhosis and hepatocellular cancer

Objectives Hydrofluoric acid is used in a number of industries. Work-related accidents involving hydrofluoric acid can result in serious injury and death. The intent of this investigation was to describe work-related hydrofluoric acid exposures in Texas. Materials and Methods Work-related hydrofluoric acid exposures reported to Texas poison centers during 2000–2010 were identified. The distribution of exposures was determined for various demographic and clinical factors. Results There were 409 work-related hydrofluoric acid exposures in years 2000–2010. The number of work-related exposures declined from 69 in 2000 to 18 in 2010. The patients were 85.3% male and 96.6% age 20 years or older. The distribution of cases by route of exposure was 69.4% dermal contact, 21.0% inhalation, 12.0% ocular, and 3.7% ingestion. At the time of reporting to Texas poison centers, the patient was already at or en route to a health care facility in 71.9% of the cases and referred to a healthcare facility in 20.5% of the cases. The medical outcome was known or expected to be not serious in 51.6% of the cases. Conclusions Hydrofluoric acid exposures reported to Texas poison centers decreased over the eleven-year period. The patients were almost all adults and primarily male. Most of the exposures occurred by dermal contact followed by inhalation. The majority of patients were managed at healthcare facilities. Slightly over half of the exposures were known or expected to not result in serious outcomes.

This article presents conclusions from analysis of the social functioning of a boy with autism in a preschool for children with special needs. The purpose of the study was to explore strengths and weaknesses in the social functioning of a boy with autism completing his preschool education. A case study was used as a research method, and observation and interview – as research techniques. The findings suggest that the social functioning of the boy with autism is at a level that allows him to continue education in an integrated or general education school as long as he is supported by a special teacher. The boy communicates his needs, has learned basic self-care skills, and follows preschool classroom rules. With adequate support from specialists and his parents, he is also able to establish healthy peer relationships.

Combating drug addiction comprises such measures as prevention, support for those already addicted (treatment, rehabilitation, reduction of health damage and social reintegration), as well as reduction of demand. In the years 2012 2013, the Supreme Audit Office carried out a comprehensive audit of the drug addiction prevention system addressed to schoolchildren and young students. The audit comprised 61 units at the central level (National Office for Drug Addiction Prevention, the Ministry of Education and the Ministry of Culture), and at the local level (city offices, municipal Police headquarters and schools), as well as selected non-governmental organisations. The audit was conducted in eight regions of Poland and it was aimed at evaluating the regularity and effectiveness of drug addiction prevention in the country. The article presents detailed results of the audit.

This paper indicates how practice (treatment, social work) may benefit from the application of notions deriving from symbolic interactionism and Situational Action Theory. I conducted interviews with therapists from a day care treatment centre. The centre offers educational assistance and counselling to children and youth aged from three to eighteen. In the article I present four methods of treatment of children and youth at risk (including juvenile offenders), which I draw from the analysis of in-depth interviews. The professionals who would benefit from applying the ideas presented in the article are probation offi cers, social workers, counsellors, street workers, and therapists.

The issue of alcohol consumption, addiction to it, and causes and effects of this phenomenon have already been explored many times. The article describes current pedagogical and legal activities taken in Poland towards people addicted to alcohol and related controversy . The authors drew attention to a number of problems concerning with alcohol addiction for an alcohol abuse person and his family. Starting from the current law on upbringing in sobriety and counteracting alcoholism because it takes up issues related to treatment, rehabilitation and social reintegration of people addicted to alcohol. There are institutions established on its basis and take up actions aimed at diagnosing the problem of an individual’s addiction to alcohol, leading to a ruling and requiring the person addicted to alcohol to undergo treatment. In the article the stages of treatment have been described in detail, requriments, important person for example probation officer and possible to receive treatment from non-governmental organizations that help those addicted and thus create an option for those addicted to alcohol to self-organize. The authors emphasized that the process of freeing from alcohol addiction is incredibly difficult and sometimes prolonged. Thus, the compulsory character of rehab treatment sparks a lot of controversy, as it contradicts the right to self-determination, i.e. independent decision of the person addicted to take up the treatment. There is a need to search new solutions in this respect, develop tools that increase the effectiveness of activities taken towards those addicted and exchange experiences on the global scale.
