Incorrect patterns of swallowing and tongue resting position and the articulatory structure of phonemes realisations in adults

The incorrect patterns of swallowing as well as improper resting postures of the tongue in adults seem to be insufficiently described in the literature. In theory, the problem is defined mainly as tongue-thrusting (interdental swallowing), in practice, however, several other abnormal patterns of the tongue’s primary motility can be also observed. The author presents not only the descriptions of incorrect patterns of swallowing and tongue at rest postures, obtained on the basis of logopaedic study conducted among 254 adults, but also the analysis of the relationship between these primary disorders and the prevalence of non-normative realisations of phonemes. The results of the profiled study which come from a profiled research group (42 patients with an incorrect swallowing pattern and tongue at rest posture but having no coexisting anatomical disorders) reveal a resemblance between the tongue’s primary motor patterns and the articulation – both in the norm as well as in a pathology. Some undesirable non-normative phonetic features of dentals and palatals such as interdentality, addentality and dorsality appear to be strictly connected with the type of an incorrect primary motoric pattern.

Nieprawidłowe wzorce połykania i pozycji spoczynkowej języka podczas oddychania u osób dorosłych wydają się niedostatecznie opisane w literaturze przedmiotu. W teorii problem określany jest głównie jako tłoczenie języka (połykanie międzyzębowe), w praktyce jednak można zaobserwować kilka innych nienormatywnych wzorców prymarnej motoryki języka. Autorka prezentuje nie tylko opisy nieprawidłowych wzorców połykania i pozycji spoczynkowej języka, uzyskanych na podstawie badań logopedycznych przeprowadzonych wśród 254 osób dorosłych, lecz także analizę związków zachodzących pomiędzy tymi zaburzeniami a nienormatywną realizacją fonemów. Wyniki badań uzyskanych w wyprofilowanej grupie badawczej (wśród 42 osób z nieprawidłowym wzorcem połykania i pozycji spoczynkowej języka, bez współistniejących zaburzeń anatomicznych) wskazują na podobieństwo pomiędzy motoryką języka podczas połykania i w pozycji spoczynkowej a motoryką artykulacyjną – zarówno w normie, jak i w patologii. Niektóre niepożądane cechy fonetyczne w realizacjach fonemów zębowych i miękkich, takie jak: międzyzębowość, przyzębowość i dorsalność wydają się ściśle powiązane z poszczególnymi zaburzonymi wzorcami motoryki prymarnej.

Autorki przedstawiają pewną transformację logopedycznego myślenia na temat ankyloglosji – sposobu badania wędzidełka języka i postępowania w przypadku jego skrócenia, a także różne opinie na temat ankyloglosji, jakie prezentują logopedzi w Polsce. W opracowaniu zaprezentowana została także propozycja typologii skróconych wędzidełek języka, będąca uzupełnieniem testu B. Ostapiuk. Autorki wskazują także problemy dotyczące ankyloglosji, które – według nich – powinny zostać rozwiązane.

The authors present some transformation of logopeadic thinking about ankyloglossia – concerning the methods of lingual frenulum examination, treatment in case of its shortening as well as different opinions expressed by speech therapists in Poland. A suggestion of a new typology of abnormal lingual frenulums, which is complementary for Ostapiuk’s test, has been presented. The authors also point further problems with ankyloglossia phenomenon, which according to them should be resolved.

The authors present the physiology of articulation of selected speech sounds in Polish. They also revise the usefulness of popular tongue exercises such as teeth counting, “cat’s back”, tongue rolling, touching the nose with the tip of the tongue, pushing the cheeks, etc. – from articulation therapy requirements. The knowledge concerning primary and secondary tongue movements as well as basic facts about tongue anatomy and physiology have also been introduced and organized in the article.
